Unexplained weight gain or loss? Here's what a blood test can check
Weight that creeps up despite no change in diet or exercise — or drops without trying — can be frustrating and worrying in equal measure. While diet, activity and sleep are usually the biggest factors, hormonal and metabolic issues can also play a real part, and several of them are simple to check with a blood test.
Common blood-testable causes of unexplained weight change include an underactive or overactive thyroid, insulin resistance or prediabetes, and hormonal shifts around perimenopause or PCOS. Each shows up clearly in the right blood markers.
This page walks through the common causes and the panels that check for them, so you can get a clinician-reviewed picture instead of guessing at the cause.
Common blood-testable causes
Thyroid imbalance
An underactive thyroid can slow metabolism and drive weight gain, while an overactive thyroid can cause unexplained weight loss. TSH, FT4 and FT3 testing can help identify or rule out both.
Check with Thyroid Profile →Insulin resistance or prediabetes
Rising insulin resistance often shows up as weight gain around the middle before blood sugar itself becomes abnormal. HbA1c, glucose, insulin and C-peptide testing can catch this early.
Check with Diabetes Status →Hormonal changes (perimenopause, PCOS)
Shifting oestrogen, progesterone and androgen levels around perimenopause or with PCOS commonly affect weight and where it's carried. A female hormone profile can help clarify what's going on.
Check with Hormone Profile (Female) →Metabolic inflammation
Metabolic syndrome — a cluster of weight, blood pressure, cholesterol and blood sugar changes — is linked to low-grade inflammation, which CRP and hs-CRP testing can flag.
Check with Metabolic Syndrome →Panels worth considering
Thyroid Profile
Your thyroid controls your energy, weight and mood. This panel checks all four key thyroid markers — much more thorough than a standard NHS TSH-only test.
Diabetes Status
This panel measures 8 markers to build a full picture of how your body manages blood sugar.
Lifestyle Screen
The Lifestyle Screen is a broad, 18-marker general wellness check covering seven areas of health in a single blood draw.
Common questions
Can a blood test explain why I'm gaining weight? +
It can help identify or rule out common hormonal and metabolic contributors — thyroid problems, insulin resistance, and hormonal changes. Diet, activity and sleep remain the biggest overall factors, but a blood test rules in or out the biochemical causes worth knowing about.
What's the best test if I don't know which panel to choose? +
The Lifestyle Screen is a good broad starting point, covering cholesterol, blood sugar, liver, kidney, iron and key vitamins in one draw. If you have specific symptoms — like period changes or feeling the cold — a more targeted panel such as Thyroid Profile or Hormone Profile may be more useful.
Do I need to fast before these tests? +
Because several of these panels include glucose, HbA1c and cholesterol, we usually recommend fasting for around 10–12 hours beforehand for the most accurate results. We'll confirm exactly what's needed when you book.
